If you don't have Adobe on your computer, click here to get it free! The University of Chicago also has a form. The university is in charge of collecting data for the MA AT Loan Program and all similar programs across the country. If you sign and send in this form, a representative from the University of Chicago will contact you 6 to 12 months after you participate in the program to conduct a brief follow-up survey. Your information will remain confidential. The only information the survey taker will have is that you participated in the program. He or she will not know whether you received a loan. By taking part in this survey you can help make sure that future grants are given to the MA AT Loan Program, which will allow the program to continue.
